Dental benefits for NCSMIG are provided via contract through a national contract with Guardian Insurance. While you are free to use any provider of your choice, your plan will pay the highest benefit if you use a provider that is contracted with Guardian. 

We utilize a Dental incentive plan. In order to keep your current incentive level, or move up a level, you will need to have at least one paid visit during the calendar year (January thru December). If you miss a year, you will be dropped one level. Incentive levels range from 70-100%.
